The manufacturer's UK product information for rizatriptan 10 mg orodispersible tablets states that the recommended dose for adults is 10 mg, that doses should be separated by at least 2 hours, and that no more than 2 doses should be taken in any 24-hour period.

The same information states that if headache returns after relief of the initial attack, one further dose may be taken within those limits, but that if a patient does not respond to the first dose, a second dose should not be taken for the same attack.

It also states that some patients should receive the lower 5 mg dose, in particular patients taking propranolol, patients with mild or moderate renal insufficiency and patients with mild to moderate hepatic insufficiency, and that rizatriptan should be separated by at least 2 hours from propranolol.

Each question fixes the values needed for its arithmetic. Write the unit beside every step and distinguish a single dose from a daily maximum or a supply period. All prescriptions and patients here are fictional; these are revision materials, not for patient care.

The method

  1. Identify the stated single dose in milligrams.
  2. Apply the stated maximum number of doses in 24 hours for the daily maximum.
  3. Check the stated minimum gap between doses before counting a second dose.
  4. Keep milligrams, doses and hours separate; convert only when the question asks.

Three practice questions

Work each one on paper first, then open the answer. All patients and prescriptions are fictional.

Question 1

A fictional patient takes rizatriptan 10 mg for a migraine attack, and the headache returns. Within the stated limits, what is the maximum amount of rizatriptan they can take in 24 hours?

Show the answer

Answer: 20 mg

Working: No more than 2 doses in any 24-hour period. 2 doses x 10 mg = 20 mg.

Question 2

A fictional patient taking propranolol is advised to use the lower 5 mg rizatriptan dose. What is the maximum amount they can take in 24 hours?

Show the answer

Answer: 10 mg

Working: No more than 2 doses in any 24-hour period. 2 doses x 5 mg = 10 mg.

Question 3

A fictional patient takes one rizatriptan 10 mg orodispersible tablet at 08:00. The headache returns, and at 13:00 they take one further dose. How many milligrams have they taken in total that day?

Show the answer

Answer: 20 mg

Working: The doses are more than 2 hours apart, so a second dose is allowed. 10 mg + 10 mg = 20 mg.

Where marks are lost

  • Counting a third dose in 24 hours even though the stated limit is two doses.
  • Ignoring the minimum 2-hour gap between doses.
  • Using the 10 mg dose when the question states a reason for the lower 5 mg dose.

Frequently asked questions

Are these official assessment questions?

No. They are original revision exercises, not official GPhC questions and not for patient care.

Does this calculation decide a real rizatriptan dose?

No. Follow the stated prescription in the exercise. Real dosing checks include attack response, propranolol use, and renal or hepatic factors.

How should I check the answer?

Check the requested unit, then work backwards from the quantity to the stated dose and duration.
